Windows
Right click on the document and select "print". Sign to complete the document.

Mac
Hover at the bottom and choose the “down arrow” to download the form to your local drive as pictured here --> 000000000 The form will download to your default download folder immediately. You can find it there. Open the downloaded document, print and sign to complete the document.

Click the "back" arrow on your browser window to return to the site.

Bring the completed form along with $8 annual dues to the next Club meeting - 3rd Wednesday, 2:30pm, Veterans Theater Kings Point Clubhouse.

You will receive your membership card at the meeting.
